Record of Complaints and Resolutions Form
Individuals who are candidates or former candidates of 含羞草研究所鈥檚 University鈥檚 Teacher Preparation Program, cooperating teachers, mentors, or representatives of a school district have the option to submit a complaint regarding the EPP for investigation and resolution. Any of these entities with a complaint can seek resolution without concern about facing retaliation. Please follow 含羞草研究所鈥檚 chain of command first, prior to completing this process. The Complaint and Resolution Procedure and Appeal Process is outlined more specifically in the聽Teacher Education Handbook.
